Contractor Requirements:

1. A copy of your Social Insurance Number or Articles of Incorporation;

2. A copy of a current (dated within 30 days of Application) Security Clearance (Criminal Record) for any and all drivers you/your company will employ;

3. A copy of a current (dated within 30 days of Application) Drivers Abstract for any and all drivers you/your company will employ;

4. A copy of your vehicle insurance certificate and policy showing that a minimum of $2,000,000 Liability coverage is carried on each vehicle you/your company will be supplying for services;

5. A copy of your vehicles registration certificate;

6. A signed Service Agreement;

7. A valid and clear Worker’s Compensation Account (Clearance Letter)
